Start by filling out the client intake form https://forms.gle/nKB5AJs5YmrKWCJj7 and set your appointment time. I will review your form before our call. During our 60 minute session we will go over the form and discuss the process and expectations.
New clients MUST submit the intake paperwork a minimum of 24 hours in advance to our session. If the paperwork has not been submitted in this time frame, I will be forced to cancel our non-refundable session.
One 50 minute coaching sessions paid and scheduled as you go.

Receive five 50 minute coaching sessions either weekly or bi-weekly. Also includes either a one 10 minute call or ask a question and follow up question via email between each session. Responses to the questions and follow up question will be answered within 48 hours.

Untying the Knot HCDC, LLC has several options for taking payments. We offer Zelle, Venmo or PayPal. Billing will not appear on your statements as Untying the Knot HCDC, LLC, but as Amber Kozak as to help maintain discretion on your financial statements.
Coaching services can be terminated by either party at any time for any reason. Upon the receipt of an email notification to cancel the services, the coaching relationship will be discontinued effective immediately. Cancelation notices must be sent to untyingtheknothcdc@gmail.com
